WHIN Enrollment

Instructions for use: Print the enrollment form. Fill out the form. Fax the completed form to WHIN at (414) 448-1202.
All forms are in PDF format — Adobe Acrobat Reader required. Get Acrobat Reader below.

	User Enrollment -Request access for an individual to WHIN-Financial, WHIN-CodeCorrect or Aurora PowerChart. Access to PowerChart requires that you are in a patient care position. Aurora Confidentiality Statement -Download if requesting access to PowerChart
	WHIN-Financial Enrollment -Request an organization have access to WHIN-Financial for eligibility verification, claims status, claims addresses, provider directories, referrals, precerts, credit information and address verification.
	Organization Enrollment -Request a new organization/account be set up with WHIN.
	Additional Location Form -Request an additional location for an existing organization be set up with WHIN.
	CodeCorrect Enrollment -Request an organization have access to WHIN-CodeCorrect.
	Patient Address Verification Enrollment -Request users access to Patient Locator to do address verification.
	User Disconnect Form -Request an existing user's login ID be deleted. This would be used when an individual leaves your organization.
